This information should not take the place of medical care and advice from your healthcare provider. What is diphenhydramine? Diphenhydramine is an antihistamine. It is found in many medicines, both prescription and free sale. It is commonly used to treat allergy symptoms, and can also be used to treat nausea, movement disease, insomnia, spicy skin, and Parkinson's disease tremor. Some of the brand names for diphenhydramine include: Benadryl®, Unisom®, Sominex® or Nytol®. I just found out I'm pregnant. Should I stop taking diphenhydramine? You should always talk to your healthcare provider before making changes to your medication. It is important to consider the benefits of treating your condition during pregnancy. Can you take diphenhydramine during my pregnancy increase the likelihood of abortion? Risk can occur in any pregnancy. There are no published studies that see if diphenhydramine increases the likelihood of abortion. This does not mean that there is a greater opportunity or that there is no greater possibility, it only means that this question has not been answered. Can the use of diphenhydramine during pregnancy cause birth defects? In each pregnancy, a woman begins with a 3-5% probability of having a baby with birth defect. This is called your background risk. Some studies have suggested a higher probability of birth defects when using diphenhydrmine during the first quarter. However, studies do not agree and no consistent pattern of birth defects has been observed. In addition, several studies did not show a higher chance of birth defects with the use of diphenhydramine in early pregnancy. Based on the current research, it is unlikely that diphenhydramine will cause a higher probability of birth defects. Can the use of diphenhydramine cause other pregnancy complications? It has not been shown that occasional use of diphenhydramine at recommended doses causes problems in pregnancy. There are rare reports of problems with the use of diphenhydramine in the third quarter. These reports often involve the use of more than is recommended or used for long-term (chronic) use of the medication. High levels of diphenhydramine could cause uterin hyperstimulation (contractions that are too long or too often). Uterine contractions may also cause severe complications, such as premature delivery, uterin rupture (a tear in the uterus) or placental detachment (when the placenta moves away from the wall of the uterus before the baby is born). There are also some reports of withdrawal symptoms (tremores and diarrhea) in babies whose mothers took diphenhydride daily during pregnancy. Is there anyone who should avoid taking diphenhydramine during pregnancy? A single human report and animal data have suggested that the combination of two drugs: temazepam (a benzodiazepine) and diphenhydramine could increase the probability of birth death or baby soon after birth. It is unknown if this interaction will occur with all medicines of the benzodiazepine class. Women who take benzodiazepine should talk to their health care provider before taking diphenhydramine during pregnancy. For more information, see the MotherToBaby data sheet about benzodiazepines in: . Can I breastfeed while taking diphenhydramine? Diphenhydramine has not been well studied for use in breastfeeding. However, some of the medications are likely to go to breast milk. Also, because diphenhydramine can cause drowsiness in adults, you can do the same for a nursing baby. If you need to take an antihistamine regularly, ask your health care provider if a medicine that does not eat medications would work for your symptoms. Antihistamines as a group could reduce the amount of milk a woman does, especially when long-term antihistamines are used and when used with decongestants such as pseudoephedrine or phenylene. This is less likely to be a matter of concern in a woman with established milk supply (she's been breast-feeding for a while). Two studies of women who used antihistamine such as diphenhydramine while breastfeeding reported that their babies were irritable, more asleep and/or less asleep. Be sure to discuss any medication you are taking and your breastfeeding options with your healthcare provider, as well as your baby's pediatrician. What if the baby's father takes diphenhydramine? There are no studies that seek to see if the use of a man's diphenhydramine would cause problems to conceive or trouble during a partner's pregnancy. A new study on unprescription sleeping aids can be a wake-up call. Published January 30, 2015 You are pregnant and cannot sleep. Miserable, ask your doctor what to take. "Diphenhydramine", you are told. It is the active ingredient in free sale sleep aids such as Benadryl, Unisom, Sominex, Exedrin PM and Tylenol PM. An antihistamine is also commonly taken for colds and allergies. The FDA classifies diphenhydramine (perhaps better known as "Benadryl") as a "Category B" drug in , which means there is no evidence of risk for fetuses. Why don't you take it? That's why not. One published in JAMA Internal Medicine found that the cumulative use of Benadryl and other similar nonprescription medications is related to and cognitive impairment in older people. This study is not the first to find a connection, but it is the largest to date. Researchers tracked nearly 3,500 people for seven years. About 800 of them developed dementia at the end of the study, and those who used Benadryl were easier to have cognitive impairment than those who did not. In particular, the study estimated that people taking at least 4 mg/day of diphenhydramine for more than three years are at greater risk of developing dementia. (The average dose of Benadryl for use as a sleeping aid is 50 mg.) The drug is an anticholinergic, which works by blocking the action of acetylcholine neurotransmitter. In the brain, acetylcholine is necessary to learn and "Wait," you might think. The participants in the study were elderly. The caution is not demonstrated. And the risk, if it's real, seems cumulative. On the other hand, this study reminds us that science has not really explored the effects of Benadryl and similar in the brains of fetuses, babies or children. Could the same anticholinergical properties have an effect on the development of a baby? If the drug contributes to dementia and loss of cognitive function at the end of life, what could the brain do at the beginning of life when its neuronal infrastructure is formed? Could the prenatal exposure appear later in life? Even a lower decrease in cognitive function would be alarming. This study ensures a good and hard look at the effects of free-sale anti-colinergic drugs on the brains developing fetuses, babies and children. If I were a doctor, I urge my pregnant and pediatric patients to consider alternatives. The fact that we now know that this drug may harm the old brain but has not yet explored its effect on the young brain... if I was pregnant again and took it, I don't think I can sleep.
